  Today's date.
  
• Name Of Your Firm:
  The legal name of your firm.
  
• Name Of Service/System:
  The name for the application that is consuming and/or displaying quotes from XigniteQuotes.
  

Locations

• Installation Address:
  The address where the servers running the application above are located.
  
• Billing Address:
  The address where NASDAQ should send the invoice for the NASDAQ Administration fee and the UTP Administrative fee.
  

Contacts

• Primary Contact:
  Contact information for the business or technical contact for the application.
  
• Billing Contact:
  Your firm's billing contact information.

• 1. Description:
  A short description of how the NASDAQ quotes from XigniteQuotes are used within your application.
  
• 2. Web Site Address:
  If the NASDAQ quotes are displayed on a public web site, enter the internet address of that web site.
  
• 3. Data Feed Provider:
  Enter Xignite, Inc. under the Provider #1 column.
  
• Account Number:
  Leave blank.
  
• 4. How will your firm distribute the market data:
  Do not check any box in the Real-Time column.
  Check the appropriate box in the Delayed column:
  - Check LAN/WAN if the data is consumed by any application but not displayed on any web page.
  - Check Internet/Intranet if the quotes are displayed on a private internet or a public web site.
  If the web site is public, enter the web site address in the corresponding field.
  If it is a password protected public web site (e.g.. investor web site), please provide a Demo ID and password.
  
• 5. UTP Market Data:
  Check both the UTP Quotation Data Feed (UQDF) and the UTP Trade Data Feed (UTDF) boxes.
  
• 6. NASDAQ Proprietary Data:
  Check the following boxes if you subscribe to XigniteQuotes:
  • OTC Bulletin Board Quotations and Trade Data
  • Other OTC Trade Data
  Check the Mutual Fund Data box if you subscribe to XigniteFunds.
  Check the NASDAQ Index Data box if you subscribe to XigniteIndices.
  
• 7. MFQS data:
  Do not check any box in this section.

• 8. Listing Market Section:
  Enter NASDAQGS, NASDAQGM, and NASDAQCM for NASDAQ.
  Enter OTCBB for OTC Bulletin Board.
  Enter OTHEROTC for Other Over-The-Counter.
  
• 9. Delay Message Section:
  Enter "Delay times are 15 mins for NASDAQ, 20 mins for NYSE and AMEX.".
  
• 10. NASDAQ Real-Time Index Data:
  Do not check any box in this section.
  

Annual Fees

• Check the Delayed UTP Data box. Do not check the other boxes.
